“why L-shaped silicone implants are not used for rhinoplasty”
Asian commonly has flat nose, so Silicone implant are ideal material for Asian nose rhinoplasty, but it must not be used on nose tip.
Because nose tip skin has very thin, and have a movable cartilages.
L-shaped silicone implant is to cover nose bridge and tip.
However, Silicone implant can undergoes calcification process under skin.
Then, the surface of nose tip becomes uneven, or can see implant a lot.
(This uneven also can happen on nasal dorsum due to extreme calcification process of silicone implant)
In severe case, when nose tip got an infection, and skin getting more thin, silicone implant can protruded.
Therefore, recommend to use autologous materials (nasal septum cartilage, ear cartilage, rib cartilage, or donated rib cartilage) for tip surgery.
This is a case of revision rhinoplasty of silicone implant removal.
This patient had tip surgery and dorsal augmentation using L shaped nose silicone implant.
However, She hated feeling hard, overly projected tip, deviated nose.
And She wanted to have a normal looking nose with good breathing.
Dr. Jin Hong Ryul conducted revision surgery for her.
L-Shaped silicone implant was removed, deviated caudal septum was corrected, tip was readjusted, and dorsum was reshaped using conchal and septal cartilage removed from the previous graft.
Now, her nose has changed to natural shape with better nose function.
